Patient Terms and Conditions for DentaQuest Dental Direct Debit Plans

1. Introduction

These Terms and Conditions govern the provision of dental services and the payment arrangements through direct debit plans offered by DentaQuest. By enrolling in the Plan, you agree to be bound by these Terms. Please read them carefully.

2. Services Covered

The Plan covers dental services as outlined in the agreed treatment plan provided by the Practice. Services may include but are not limited to regular check-ups, cleanings, X-rays, and other dental procedures as recommended by the dentist. Routine dental examinations are included as part of a guardian’s plan for 1 child under the age of 16.

3. Enrolment and Payments

– To enrol in the any of our plan’s patients must have a new patient exam which is £75, we may discount the cost of this initial appointment from time to time however this is at the discretion of the practice.

– Patients must complete and sign the enrolment form provided by the Practice and pay the one off £8.95 administration fee.

– By enrolling, patients authorise the Practice to collect payments via direct debit from the nominated bank account or credit/debit card.

– Payments will be collected on a recurring basis as agreed upon in the enrolment form, monthly.

– The Practice reserves the right to adjust the payment amount with prior notice to the patients.

4. Cancellation and Termination

– Patients may cancel their enrolment in the Plan with no cancellation fee if they cancel within 14 days of joining, however if they have redeemed a service in this time the difference must be paid.

– To cancel their enrolment in the plan patients must provide written notice to the Practice.

– Plans are subject to 12 month rolling contracts after the cancellation period of 14 days.

– The Practice reserves the right to terminate a patient’s enrolment in the Plan due to non-payment or breach of these Terms.

– Upon cancellation or termination, the patient remains responsible for any outstanding payments owed for services received.

5. Changes to Services or Terms

– The Practice reserves the right to modify the services covered by the Plan or these Terms with reasonable notice to the Patient.

– Patients will be informed of any changes to the Plan or Terms via email or written notice.

6. Confidentiality

– The Practice will maintain the confidentiality of all patient’s information in accordance with applicable privacy laws and regulations.

– Patient information will only be used for the purpose of providing dental services and managing the Plan.

7. Liability

– The Practice will not be liable for any loss, damage, or injury arising from the provision of dental services, except to the extent caused by our negligence.

– Patients are responsible for maintaining appropriate insurance coverage for dental treatments not covered by the Plan.

8. Governing Law

These Terms shall be governed by and construed in accordance with the law. Any disputes arising under these Terms shall be subject to the exclusive jurisdiction of the courts.
